Disclaimers regarding the VIP Cosmetic Network:
- the VIP Cosmetic Network is NOT an insurance plan;
- we provide discounts at certain health care providers for medical cosmetic procedures and medical grade skin care products;
- we do not make payments directly to the heath care providers of medical cosmetic procedures and medical grade skin care products; and we make no representation or warranty regarding the manner, method or outcome of any medical cosmetic procedures or medical grade skin care products provided by any health care provider participating in the Network and we have no liability to you from any decision made or given by any heath care provider participating in the Network.
Membership Terms:
1. To receive any medical cosmetic procedure or medical grade care product at a discount from a health care provider in the Network, I must show my VIP Cosmetic Advantage Card, along with a photo identification, to the heath care provider at the time the procedure is provided or the product is purchased.
2. Only the member and, if applicable, the member’s spouse, to whom a VIP Cosmetic Advantage Card is issued may use the VIP Cosmetic Advantage Card to purchase a medical cosmetic procedure or medical grade skin care product.
3. I am obligated to pay for all medical cosmetic procedures and medical grade skin care products at the time the service is provided or the product is purchased.
4. My membership is effective upon (i) my submission of this completed, signed and dated enrollment form, (ii) payment in full of the yearly fee of $199 for my participation in the Network, or, in the event my spouse participates, payment in full of the yearly fee of $398 for my and my spouse’s participation in the Network, and (iii) issuance of the VIP Cosmetic Advantage Card. My membership and, if applicable, my spouse’s participation, in the VIP Cosmetic Network will terminate twelve months from the effective date of my participation unless I renew my (and/or my spouse’s) membership.
5. I release VIP Cosmetic, Inc. and the VIP Cosmetic Network from any and all liability, including but not limited to consequential or incidental damages, to myself or any third party from any decision made or given by any VIP Cosmetic Network health care provider who provides a medical cosmetic procedure or medical grade skin care product to me.
6. VIP Cosmetic, Inc. may revise the yearly fee for my participation or, if applicable, my spouse’s participation, in the VIP Cosmetic Network on a prospective basis upon 30 days written notice to me.
